Foundation for Senior Care

Our donation-based, rides program offers in-town rides, grocery deliveries, and expanded, out-of-town rides –
for health-related appointments only -- within a 50-mile radius of Fallbrook.
In-town rides might include trips to medical appointments, pharmacies, dentists, grocery stores, hair salons, or
a friend’s house. Our friendly drivers provide door-to-door service in a comfortable Care Van and can offer
assistance in and out of the vehicle. Wheelchair-accessible transportation all around town is also available.

Expanded, out-of-town rides need to originate in our service area, must be within a 50-mile radius of Fallbrook,
and must be for health-related appointments only. Typically, one of our dedicated volunteer drivers will provide
this door-to-door service using their own vehicle. They will wait for you (up to 3 hours) while you are at your
appointment.
All rides and grocery deliveries are subject to driver availability and must originate in our service area.

The Foundation for Senior Care serves seniors, and adults with disabilities, in Fallbrook, Bonsall, Rainbow, and De Luz. Additionally, our transportation program offers donation-based, expanded rides to areas within a 55-mile radius of Fallbrook proper. We also offer round-trip transportation to and from our Adult Day Care Center for clients in the Temecula area.

Service Hours: Monday – Friday: 9:00 am – 4:00 pm

Suggested Donations:

• $10 for round-trip rides or grocery deliveries within our service area
• $45 for round-trip, expanded rides within a 50-mile radius of Fallbrook, including North San Diego County and Southwest Riverside County. Out-of-town rides must originate in our service area and be for healthcare-related appointments only
• $60 for out-of-town wheelchair transportation for healthcare-related appointments only. Our staff drivers provide this service and it typically takes two to four hours of their time
